• Please check our “Insurance List” , on the left hand side, to see if we are contracted with your insurance carrier. If your insurance carrier is not listed we offer a Cash Pay plan as well; please contact our office for more information.
   
• If you are interested in being treated at our facility you may further utilize our website to actually print out our patient information packet, listed on the left hand side under “New Patients” , prior to your appointment. Simply fill out all of the forms completely, fax to 702-368-6775 or bring them to our office.
   
• Once your information has been sent to our office we will contact your insurance carrier to receive authorization for treatment and to find out if you, the patient, are going to be financially responsible for any payments. At this time we will contact you to go over the benefits that were obtained and to set up the appointment with a physical therapist.
   
• We also suggest emailing us if you have any additional questions using the “Contact Us” page, someone will respond back to you with in 24 hours.
